What is Youth Pathways?
Youth Pathways helps young people get through school and make the transition from school to further education, training or employment. Youth Pathways delivers personalised assistance to meet individual needs.
Am I Eligible for Youth Pathways.....
Youth Pathways will identify participants who are:
- at risk of leaving school before completing year 12 and at risk of not making a successful transition through school and from school to further education, training or employment OR
- have left school in the preceding 12 months without completing year 12.
How Does Youth Pathways Work.....
Youth Pathways participants will received personal assistance and activities specifically directed to their needs:
Activities include:
- Identification of personal barriers to participation
- Development and implementation of an individually appropriate Transition Plan
- Individualised case management designed to address the barriers identified
- Group work
Assistance includes:
- Problem
- Motivation
- Skills development
- Counselling
- Mentoring
- Advocacy
- Mediation
- Assistance access to education
- Training and employment services
- Referral to specialist support services such as Youth Suicide, drug and other community activities
How Do I Find Out More About Youth Pathways.....
- Youth Projects will take an active approach to managing and empowering participants and utilise a wide range of early intervention and case management strategies.
- Youth Projects have good links with community, schools, knowledge of local education and training that relate to young people.
